Please fill out the following in the Instrument Checkout form in order to receive an instrument from the school:

2) Instrument
3) Check the boxes indicating you will also be receiving a "bow" and "case"
4) Address
5) Phone number
6) In lieu of a physical signature, please email the form to Harrison.Cook@cobbk12.org and say "I grant (Student) permission to receive this instrument from the school and accept responsibility for it."
